Please be sure to provide your name, email, and phone number so that we may contact you should there be an issue with your submission. Please note that ALL submissions are subject to review by MADD Canada staff before being published to our website.

MADD Canada reserves the right to decline to publish tributes, messages, and photos at our own discretion. MADD Canada is not responsible for inaccuracies or other issues with tribute submissions. If you see an inaccuracy with one of the tributes, please contact us.
